Kindergarten Registration

Kindergarten Registration for the 10-11 school year
will be March 15-March 19, 2010
8:00 a.m. - 3:00 p.m.

At the time of registration, parents must have the following information:

• Birth certificate - child must be 5 years of age on/or before September 15, 2010
• Immunization Record - current immunizations must include:
  5-DPT, 4-Polio, 2-MMR, 3-Hepatitis B, 2-Varicella (chicken pox)
• 2 Proofs of Residency - utility bill, lease agreement, real estate contract, etc. (we cannot accept a driver's license)
• Current Phone Numbers - phone numbers for home, work, two emergency contacts, and physician
  
  We CANNOT accept incomplete registration paperwork. Please ensure that you have all the items listed above when you come for registration.
  
  Registration packets will be available at the school after March 1.
